      为探究飞秒激光小切口角膜基质透镜取出术(Small incision lenticule extraction,SMILE)术后角膜的变形和应力变化,本文采用 Odgen 超弹本构方程来描述角膜力学行为,构建角膜有限元模型,在 14、16、18和 20 mmHg 四种眼内压作用下仿真 SMILE 角膜屈光手术矫正屈光度(-1~-6 D)、中央角膜厚度(480~630μm)、角膜切削直径(4~8 mm)对术后角膜内表面中央顶点位移差的影响,并研究术后角膜内表面整体位移和等效应力变化。结果表明,在相同眼内压作用下,角膜内表面中央点位移差与矫正屈光度呈近似线性增长;中央角膜越厚,角膜内表面中央点位移变化则越小;角膜切削直径越大,角膜内表面中央点位移差变化越显著。对中央角膜厚、矫正屈光度低、切削直径小的患者,行 SMILE 术后,角膜变形较小,手术安全性高,术后不易诱发角膜扩张膨隆。本文工作可为临床近视眼角膜 SMILE 屈光手术术前准备和术后效果预测提供参考。

      In order to explore the change of deformation and stress of the cornea after the small incision lenticule extraction(SMILE),an Odgen superelastic constitutive equation was used to describe the mechanical behavior of the cornea,and corneal finite elementmodels were constructed before and after SMILE. Under the intraocular pressures (IOPs) with 14、16、18 and 20 mmHg,theinfluence of refractive powers(-1~-6 D),central corneal thicknesses(480~630 μm),and ablation diameters(4~8mm)onthe displacement difference of the central vertex of the postoperative corneal inner surface was analyzed,and the overall cornealdisplacement and equivalent stress changes of postoperative cornea were studied. The results showed that under the same IOP,thecentral displacement difference of inner surface of cornea increased linearly with the corrected diopter; the thicker the central cornea, the smaller the central displacement of the inner surface of cornea,and the larger the diameter of corneal cutting,the more significantthe change of central displacement of corneal inner surface.For patients with thick central cornea,low corrected diopter and smallcutting diameter,corneal deformation is small after SMILE operation,and the operation safety is high. Furthermore,the cornea is noteasy to induce dilatation and swelling after operation.For clinical myopia,this work can provide reference for preoperative preparationand postoperative prediction of corneal SMILE refractive surgery .
